Freeport Medical Center is proud to be one of the few dialysis centers in the region approved and endorsed by the Ministry of Health. Equipped with over 22 state-of-the-art dialysis machines, we are fully prepared to meet your care needs with excellence and compassion.

We also have a dedicated quarantine unit for patients who require isolation.

• Required Medical Documents:

  HIV Test Results:

  HIV 1 & 2 test results dated within the last 3 months.

  Hepatitis Test Results:

  Hepatitis B and Hepatitis C test results dated within the last 3 months.

  Medical Letter:

  A letter from the patient’s nephrologist detailing the current medical condition.

  Dialysis Unit Documentation:

  Three recent flowcharts or treatment records from the current dialysis center.

  Medication List:

  A current list of all prescribed medications.

  Vaccination Record:

  Complete and up-to-date vaccination record.
